Before you begin your examination, the grandson, an attorney, tells you that elderly Italianwomen are extremely modest and might not answer questions completely. He indicates thathe'd like to stay in the room during the examination

How will you proceed?

First have the interpreter ask her permission for the grandson to stay in the room for the history
portion; however, he would need to step just outside the room for the physical examination. It
would be a violation of the Health Insurance Portability and Accountability Act (HIPAA) to have him
stay without the patient's permission. Again, before asking her a personal question, try to establish
rapport. Watch her as she answers the interpreter's questions, and watch her body language. Explain
before asking the questions that some of the questions might seem "personal" or "embarrassing"
but that the information will be important to know to help her to feel better.
