Ans: Inflammatory Bowel Disease (IBD) is a disease that is caused due to the malfunctioning of the host immune system against the gut microflora, which leads to damage of intestinal lining, ulcers, edema, and loss of fluids and electrolytes. Complications of IBD can be either localized or systemic.

Fever is a response of the body to ward off bacterial infection. The immune system plays a large role in thermoregulation of the body during response to infections.Immune cells release chemicals called as cytokines, which stimulate the hypothalamus to increase the core body temperature.

In the case of C.W., her immune system is already supressed by the high doses of mesalamine and prednisone taken over the years. C.W. has basically become immuncompromised against infections. This is one of the reasons why C.W. is finding it difficult to cope up with her wound healing and increasing her core body temperature as desired.

Moreover, the lack of proper nutrition and metabolic fuel is hindering the production of ATP molecules, which are required for increasing the core body temperature in fever. Additionally, the C.W.'s care giver needs to ensure that she does not develop sepsis, which is common in case of invasive surgical procedures.

The symptoms of sepsis can also overlap with the present symptoms exhibited by C.W., which may make it difficult to proper,y diagnose any underlying condition. C.W. needs to be treated for any infection immediately with broad range antibiotics in order to prevent further spread of infection.
