Opioids have two effects, analgesic at lower doses and suppression of respiration at higher doses. These two effects each have a sigmoidal dose-response, with the respiratory effects being only minimal at relatively high analgesic doses. When a person develops tolerance for the analgesic effects, the response is to increase dosage. As dosage is increased, a person can find inhibitory effects on respiration. Since the respiratory effects are when there has already been suppression of pain, the individual is unaware of depressed respiration and can suffocate.
