The employer has certain obligations to report injuries and illnesses, but not all must be reported to federal or state authorities. Two categories exist in which recorded cases must be reported: 1) audits of recorded injuries or illnesses for a certain time frame, requested by federal or state health and safety organizations, and 2) all accidents resulting in one or more fatalities or hospitalization of three or more employees. Some states have more stringent reporting requirements. The employer must orally report within 8 hours work-related fatalities and incidents involving the hospitalization of three or more employees to the nearest OSHA office or the OSHA Hotline at 1-800-321-OSHA. Again, some states have more stringent requirements; for example, CAL/OSHA requires immediate notification of a death or serious injury or illness.
