What happened at the last case review meeting? What do you think about Emeliaís report on the Snyders? Did Emelia make good use of her interdisciplinary team?

Emelia coordinated a Meals on Wheels Program for the elderly, and supervised 10 volunteers who delivered the meals. One day, when Emelia filled in for a volunteer and delivered a meal to the Snyders, she found Mr. Snyder very agitated and worried because Mrs. Snyder was missing. Apparently she had wandered off before. Because it was cold outside and they lived two blocks from a busy street, Emelia was alarmed and struggled with how to respond to this unexpected situation. As she tried to prioritize her responsibilities, she began to wonder if her volunteers were communicating with her and if they were well-trained for their responsibilities.

Emelia reported at the last case review meeting that everything seemed to be However, she should have mentioned several things that might be of concern. Mrs. Snyder mentioned having more difficulty with her arthritis and that she was taking a new medicine for it that did not seem to be helping. These changes suggest a need for an assessment and monitoring of the new medication. She might also have mentioned how thin Mr. Snyder appeared the last time she saw him, and discuss the need to monitor how well he was managing his diabetes. The case review team could have discussed these issues and offered more insight. With their expertise in different fields, they could have helped Emelia develop a new plan to meet the Snydersí needs. In addition, Emelia is relying on her volunteers to raise any medical concerns, which may mean she is missing information.
Emeliaís brevity may have been due in part to her limited work and life experience and her failure to understand the significance of certain changes in the Snyders.
