Assess doll's eyes phenomenon (oculocephalic reflex) in the unconscious patient. This tests the intactness of the vestibular and oculomotor pathways. Doll's eyes phenomenon should not be tested in the patient with a suspected neck injury. Hold the patient's eyelids open and rotate the head of the patient from the center to one side and then to the opposite side, holding briefly at the end points. Watch for eye movement. Normally, the eyes should deviate in the direction opposite the head movement. Turn the head only from one side to the other; the test does not involve raising or lowering the bed, nor is pain or light stimulus indicated.
