The caption has the docket number, the names of the parties, the court name, the geographical location, and date of filing.
The body identifies the movant (party filing the motion), the relief sought, and the grounds/basis upon which this relief is requested.
The signature or subscription section gives the designation of the party in the lawsuit as to whether plaintiff or defendant, the name of the party, a signature line, the name of counsel or pro per or pro se, the address of either the attorney or party, and telephone number. It the party is represented by counsel, the attorney's license or bar number also must be included.
There may need to be an affidavit of fact appended.
The order also may be included depending on the rules of the jurisdiction or court preference. It is a statement that sets forth the judge's decision on the motion.
