Suggested response: In John's case, we are told he has not eaten today and has drunk very little; however he has vomited. So we would use a rapid sequence induction, but avoiding drugs that are more likely to cause/worsen haemodynamic instability or modify the dose. So a drug such as thiopentone would be avoided; judicious use of propofol or midazolam and morphine should be considered, for example a lower dose to avoid exacerbating low blood pressure. Formula for tube size is (age in years divided by 4) plus 4, so a tube size of 6.5 mm would be ideal, with the cuff inflated to an ideal pressure between 5 and 15 cmH2O and no higher than 20 cmH2O.
