The nurse is caring for a 4-year-old boy who has undergone an appendectomy. The child is unwilling to use the incentive spirometer. Which approach would most likely elicit the child's cooperation?

A) "Can you cough for me, please?"
B) "You must blow in this or you might get pneumonia."
C) "If you don't try, I will have to get the doctor."
D) "Can you blow this cotton ball across the tray?"

D
Response:
Children are more likely to cooperate with interventions if play is involved. Encourage deep breathing by playing games. Asking the boy to cough is less likely to engage him. Telling the child he might get pneumonia is not age-appropriate and unhelpful. Threatening to call the doctor is unhelpful and inappropriate. Remember, however, that the incentive spirometer works on the principle of the amount of air inhaled, not exhaled. Having the child take a deep breath prior to blowing the cotton ball is a beginning step.

A 20-year-old patient who had an open appendectomy this morning has just been admitted to the postsurgical unit from postanesthetic recovery. The patient has a persistent cough that is likely a result of airway irritation caused by intubation

The patient has asked the nurse for something the help with his cough and the nurse is planning to ask the patient's physician for an order for codeine. The use of codeine is contraindicated in this patient because A) the resulting GI upset will increase his chance of developing paralytic ileus. B) codeine will not resolve coughing that is the result of physical airway irritation . C) codeine cannot be administered less than 48 hours after general anesthetic. D) codeine will make it more difficult for him to maintain a patient airway because of his post-operative status.
