The methods used by supervisors may vary according to their individual needs. Some agencies have adopted an incident report form to be used to record commendable or censurable performance of a minor or routine nature. These reports should be retained during the rating period, as they form a substantial basis for performance evaluations. They should then be placed in the employee's personnel folder or destroyed. Some supervisors prefer to record observations in a separate file for future reference; others record incidents in the supervisor's log and later transfer these notations to a file or a control card kept for each officer.
