function; maximize communication; help Y.W. relearn activities of daily living; provide bowel
and bladder retraining; have physical therapy plan an exercise program to regain balance and
muscle strength; and provide psychological support through family, friends, and others. Provide
education and support for Y.W.'s family and any caregivers. Be cognizant of the impact of any cultural
differences. Identify which services Y.W. will need, then determine which care setting will be most
appropriate. Discharge cannot occur until it is reasonably certain that Y.W. will be in an environment
in which his health care needs will be met.
