The medical coder traditionally was responsible for identifying inaccurate, nonprecise, or unsubstantiated information in the medical record in the course of assigning codes to diagnoses and procedures. They were tasked with the role of communicating with the physician or other party responsible for the documentation and facilitating the improvement of the documentation. In the future, the coder's role will increasing be automated by computer-assisted coding, and the role will increasingly become more focused on verification as well as quality checking of documentation.
