Almost all of the research on causes of various kinds of gender variance has been done on transsexuals. There are some inklings of biological factors. One line of thought is that processes in prenatal development are involved, in which the genitals differentiate toward those of one gender and identity differentiates toward the other gender. Two studies have found differences between male-to-female (MTFs) transsexuals and typical men, in the bed nucleus of the stria terminalis (BST), which is part of the limbic system. In other MRI studies, FTMs display brain regions that are intermediate between cismales and cisfemales. Other research has identified several genes associated with transsexualism, and the genes are different for MTFs and female-to-male transsexuals (FTMs).
The bottom line is that, right now, we do not know the exact causes of transsexualism or other kinds of gender variance.
