Extrapyramidal rigidity is characterized by
a. sustained contraction of extensor muscles with extension of legs, arched neck, and clenched teeth.
b. sustained contraction of flexor muscles, with hyperflexion of the arms and legs.
c. sustained spasticity and rhythmic oscillation of involuntary muscle contraction.
d. resistance to passive movement of flexor and extensor muscles in all directions.
D
Extrapyramidal rigidity is evident when resistance is present during passive movement of the muscles in all directions and lasts throughout the entire range of motion. It can involve both flexor and extensor muscles and is abnormal. Decerebrate rigidity (decerebration) is characterized by rigidity and sustained contraction of the extensor muscles with extension of the legs, arched neck, and clenched teeth. Decorticate rigidity (decortication) is characterized by hyperflexion of the arms and hyperextension and internal rotation of the legs. Sustained spasticity and rhythmic oscillation of involuntary muscle contraction is not indicated in extrapyramidal rigidity.

During the nurse's first assessment upon admission, the findings are: blood pressure 90/60 (as compared to 136/66 in the emergency department), flaccid paralysis on the right side, absent bowel sounds, zero urine output, and palpation of a distended bladder.
