Explain the etiology of ear infections.
OM—inflammation of the middle ear—is one of the most common early childhood illnesses.
Acute OM can be caused by a viral or bacterial source. Many factors predispose infants and young
children to OM.
• The eustachian tube (ET) is underdeveloped, short, and horizontal, lending itself to blockage and
reflux of milk into the ET during feeding.
• Young children have an underdeveloped immune system.
• Enlarged pharyngeal lymph tissue (tonsils and adenoids) contributes to obstruction of the
eustachian tube.
• Infants are in a prolonged lying position, contributing to pooling of fluids.
