The MFM recording algorithm generates a clock pulse at the boundary of each cell (recorded bit). There is a pulse in the cell if that bit is a 1 and no pulse if it is a 0. That would leave no signal if two 0s appear together. In this case an extra pulse is inserted between two consecutive 0s. The final signal is generated by making a transition in the head current at each pulse.
