Discuss the provisions of the Individualized Family Service Plan (IFSP)

ANSWER: The IFSP includes goals and objectives for the young child with disabilities and for the
family. The IFSP must contain statements pertaining to 1) the child's present levels of
development (cognitive, speech/language, psychosocial, motor, and self-help)? 2) the
family's concerns, priorities, and resources related to enhancing the child's development?
3) major outcomes expected to be achieved for the child and family and the criteria,
procedures, and time frames for determining progress? 4) specific early intervention
services necessary to meet the unique needs of the child and the family? 5) projected
dates for initiation of services and expected duration? 6) the case manager who will be
responsible for implementing the plan? and 7) procedures for transition from early
intervention into a preschool program.
