What is the proper specimen for culture and identification of the agent of acute sinusitis?

a. Aspirated material that has been collected by puncture of the maxillary sinus
b. Drainage material found in the upper res-piratory passages that has been collected with a swab
c. Coughed up sinus drainage collected in a sterile cup
d. Sinus drainage

A
In most cases, a diagnosis can be made on the basis of physical findings, history, radiographic studies, and other imaging techniques such as magnetic resonance imaging. However, if a labor-atory diagnosis is needed, then an otolaryngologist obtains the specimen from the maxillary sinus by puncture and aspiration or during surgery. Sinus drainage is unacceptable for smear or culture; such a specimen will be contaminated with aerobic and anaerobic normal respiratory flora.
