Barriers in work with Behavioral Health Care settings is that often primary health care and behavioral health care are geographically separated. Set in different buildings, the integration of services may be encumbered by lack of referral from primary care to behavioral health, lack of communication between the offices and lack of follow up. The best remedy is to have the services within the same facility, with communication systems set up between providers and follow up given within a timely manner. Benefits include the potential for longer independence in living, fewer hospital readmissions and improved health status.
