Fat embolism syndrome is a rare, but potentially life-threatening, complication that accompanies
traumatic injury of the long bones, such as the femur, and pelvis. If it occurs, it is within 48 hours of
the injury. In fat embolism, fat globules are released from the marrow of the broken bone into the
bloodstream. Once the fat droplets enter the circulation, they migrate to the lungs. Because they
are too large to pass through the pulmonary circulation, they lodge in the capillaries and obstruct
blood flow. The fat particles break down into fatty acids, which inflame the pulmonary blood vessels,
leading to pulmonary edema and the occlusion of capillaries of the pulmonary circulation, causing
hypoxia and tissue death.
