a. Ask as few questions as possible and never ask for information that you can obtain on your own.

c. Ask yes-or-no questions or short-answer questions when possible; however, use open-ended questions when you need more information.
d. Arrange your questions in a logical order.
e. Word each question clearly and objectively.
f. Limit the content to one topic per question.
g. Assure the reader that his or her answers will be treated confidentially, if necessary.
