For paper claim submissions, actual provider signatures are acceptable. If an actual signature is not used, the statement "SIGNATURE ON FILE" may be used as long as the practice or facility maintains a current signature file that has the written signature of every provider or physician. Signature stamps may be used when an arrangement has been made with the insurance carrier to accept the stamp.
