Dialysis disequilibrium syndrome often occurs after the first or second dialysis treatment or
in patients who have had sudden, large decreases in BUN and creatinine levels because of the
hemodialysis. The rapid removal of urea from the blood leaves a higher concentration of solutes in
the brain and the cerebrospinal fluid (CSF). The solutes increase the osmotic pressure, which draws
fluid from the bloodstream into the CSF and brain tissue. The extra fluid in the brain tissue creates a
state of cerebral edema for the patient, which results in severe headaches, hypertension, nausea and
vomiting, twitching, mental confusion, lethargy, restlessness, and occasionally seizures.
