There could be a number of reasons for a code of ethics or conduct failing to have
an effect in the workplace. The most obvious is communication. If there has not been a careful strategy implemented to make employees aware of--and understand--the code, then management can’t expect them to incorporate it into their working lives. Just making the code available isn’t enough; management could discuss it in training workshops, announcements, link knowledge of it to promotions and awards, and many other actions.
Additionally, if the code was developed without employee input, they could feel
disconnected from its provisions. They might assume that the code was adopted only because management felt it was legally required or that they were publicly pressured to do so. Finally, understand that having a code of ethics or conduct is just one component of an ethical culture--it must be combined with other elements of an ethical culture to have an effect.
