If there is a breakdown of the nursing process, patients are at high risk of negative outcomes and the facility and its personnel are at high risk of legal exposure. In order for patients to receive proper care, the nursing assistant must understand her responsibilities for observing patients and reporting the information, as well as following the information in the care plan to meet each patient's individual needs.
