The physician orders the following for T.W.: Intravenous (IV) methylprednisolone (Solu
Medrol), bolus of 30 mg/kg over 15 minutes, followed by a maintenance infusion of 5.4 mg/kg
body weight per hour. T.W. weighs 176 pounds.
How many milligrams of
methylprednisolone (Solu-Medrol) will T.W. receive with the bolus? How many milligrams
per hour will T.W. receive with the maintenance infusion? What effect will this medication
have on T.W.?
176 pounds/2.2 = 80 kg× 30 mg/kg = 2400 mg for the initial dose; the maintenance infusion will
then be 5.4 mg/kg× 80 kg, or 432 mg/hr.
Methylprednisolone (Solu-Medrol), a steroid or anti-inflammatory medication, administered at
high doses within 8 hours of an SCI is effective in reducing inflammation and subsequent damage
to nerve cells. An adverse effect of therapy is it will render him more vulnerable to hospital-acquired
infections. The duration of the steroid infusion depends on when therapy began: If treatment was
initiated within 3 hours of injury, the infusion is administered for 23 hours. If treatment began
between 3 and 8 hours, then infusion is continued for 47 hours.
