If the anticipated central venous catheter (CVC) dwelling time is prolonged more than 5 days, the subcutaneous (SC) site is preferred. The SC position has the lowest infection rate and produces the least patient discomfort from the catheter. The internal jugular (IJ) vein is the most frequently used access site for CVC insertion. Compared with the other thoracic veins, it is the easiest to canalize. If the IJ vein is not available, the external jugular (EJ) vein may be accessed, although blood flow is significantly higher in the IJ vein, making it the preferred site. This may be the reason why catheter-related infections are higher in the IJ than the SC position for indwelling catheters left in place for more than 4 days. The femoral vein is considered the easiest cannulation site because there are no curves in the insertion route. Because there is a higher rate of nosocomial infection with femoral catheters, this site is not recommended.
