To calculate maintenance fluid needs, use the following formula (Hockenberry MJ and Wilson, D, 2013):
1) The first 10 kg of weight × 100 mL
2) 11 to 20 kg of weight × 50 mL
3) Over 20 kg of weight × 20 mL (The weight remaining after the first 20 kg)
Thus, for R.O.'s weight of 42 kg:
1) For first 10 kg: 10 × 100 mL = 1000 mL, plus
2) For second 10 kg: 10 × 50 mL = 500 mL, plus
3) For remaining weight above 20 kg: 22 × 20 mL = 440 mL (note that 42 kg − 20 kg = 22 kg)
1000 + 500 + 440 = 1940
Total maintenance needs are 1940 mL in 24 hours
R.O. is NPO so this amount will need to be met through IV fluids. The setting on the infusion pump
should be 80.8 or 81 mL/hr. The previously mentioned IV order is appropriate. Once she returns from
surgery and is taking fluids, the rate would be adjusted.
