Nursing diagnosis is the second step in the Nursing Process. It is the link between the preceding assessment data and all future phases. It further provides the basis for planning client-centered goals and interventions. When possible, the nursing diagnosis as well as all other steps in the Nursing Process should be validated with the patient. The diagnostic statement is written after all data are collected and reflects the nurse's clinical reasoning in establishing the nursing problem. Nursing diagnoses are human responses to health problems, whereas medical diagnoses establish disease processes.
