Rhesus isoimmunisation can only occur between a mother who has a negative blood group, and a baby who has a positive blood group. • A Kleihauer-Betke test should be performed within 2 hours of birth. The test is able to determine the number of fetal cells present in the maternal circulation. • If the baby's blood group has already been determined, and it is positive, then Daisy should receive 625 IU of Rh D-Ig as an intramuscular injection before the KleihauerBetke test result is received for prophylaxis, as she has had a ‘sensitising' event for isoimmunisation by giving birth to a positive blood group baby. • When the test results are received, Daisy may require an additional dose of Rh D-Ig; however, the dose required is dependent on the number of fetal cells detected. A standard 625 IU dose will cover a 6-mL fetal haemorrhage. • A repeat Kleihauer-Betke test should be performed 48 hours after prophylaxis in women with a fetal cell count >240 to ensure effective prophylaxis.
