During your shift, every 15 minutes you check the patient and he reports numbness in his right hand. What do you do?

Release the affected wrist. Allow the patient to do ROM and see if the numbness and tingling goes away. Replace the restraint following manufacturer's instructions and make sure the restraint is not too tight. A finger should easily fit between the restraint and the wrist. Notify the nurse immediately.
