Answer: Sexual stimulation in the male activates the chemical cGMP, which relaxes smooth muscle in the arteries of the penis. This increases blood flow to the penis, and creates an erection. Afterwards, the enzyme PDE5 metabolizes cGMP, and the erection subsides. PDE5 inhibitor drugs inhibit the enzyme PDE5 and so cGMP is not metabolized and the erection is sustained. Prostaglandin E1 drugs act locally to relax the smooth muscle in the arteries of the penis, increase blood flow to the penis and create an erection.
