F.F. weighs 277 lb = 125.9 kg. The dose is 6 mg/kg/day = 125.9× 6 = 755.4 mg/day. Divided doses
q8h = 3 doses in 24 hours (24/8 = 3). 755.4 mg/day in divided doses q8h = 755.4/3 = 251.8 or
252 mg/dose.
The recommended dose for gentamicin is 3 to 6 mg/kg/day if there are no renal concerns. The
ordered dose of gentamicin is at the maximum dose and has the capacity to be nephrotoxic. F.F.'s
renal function is already impaired, as reflected by the elevated BUN and creatinine. The dose of the
aminoglycoside gentamicin will vary, depending on the patient's renal function.
