Sotalol rather than amiodarone was chosen as the first antidysrhythmic agent used for Mr Thomas. What reasons might have prompted this decision?

What will be an ideal response?

Suggested response: Of primary importance is that beta-blockers reduce mortality in the post infarc tion population. Therefore, an antidysrhythmic with beta-blocker effects pushes the antidysrhythmic decision towards sotalol. Amiodarone is a superior antidysrhythmic to sotalol but the risk of serious side effects over a possible lifetime of treatment in a young man becomes important. Finally, Mr T worked outdoors and for longer hours in summer. His recreations were also outdoors and the risk of photosensitivity and dermal changes from amiodarone were thus very relevant to him.
