perform cognitive skills above her level of ability; reduce environmental stimuli to decrease
overstimulation; develop and maintain daily routine; and provide continuity of care.
Self-care deficits: Continuously assess M.E.'s self-care needs and her ability to participate in her
care. Help maintain independence by having her do as much as possible, reminding her how to
perform self-care and ADLs as needed; provide direct care as needed.
Disrupted sleep pattern: Control her activities so she is awake during the day; provide measures
to facilitate sleep at night. Maintain an environment conducive to sleep at night. Administer
appropriate medications (e.g., lorazepam [Ativan] for anxiety).
Impaired verbal communication: Develop alternative patterns for communication, watch for subtle
communication clues, anticipate her needs, and make certain you get M.E.'s attention when
speaking to her.
Agitation: Maintaining consistency with day-to-day activities is the paramount consideration in
controlling agitation.
