Systems request forms typically ask for information that will assist a systems review committee in identifying the nature of the problem, determining the priority of the issue and information that will assist in conducting an initial feasibility study. Questions have to determine if the request is for a new system, improvement in a system, or to correct an error in an existing system. Questions that will help determine the urgency may include asking if there are workarounds in place and is the current system losing money. The request has to ask for a detailed explanation of the problem and what the requestor sees is a possible solution.
