In some cases, the end-user makes inappropriate use of the system, causing the failure. In other cases, the system designers deserve the blame because they produced an unsafe design. Introduction of autonomy adds another wrinkle to this problem, because, in autonomous systems, the end-user is essentially a bystander in the decision-making process.
