Unlike non-smokers, whose basement membranes are in general not compromised in pulmonary alveoli, smokers have alveoli that are damaged as a result of chronic exposure to cigarette smoke. Damage provides a gateway by which autoantibodies can gain access to basement membranes, where they become deposited, activate complement, and cause rupture of alveolar blood vessels.
