A skilled nursing home unit is in such demand that it always is full. A full unit requires a staff of 15 (8 day, 5 evening, and 2 night shift workers) every day. Explain why the nursing home will need to hire more than 21 FTE workers.
What will be an ideal response?
The unit requires 15 workers per day, 7 days per week, 52 weeks per year which is 21 FTEs. However, employees get vacations and sick days. If the average full-time employee receives 3 weeks (15 days) of vacation per year and uses 5 sick days per year, then coverage is needed for 20 work days (160 work hours) per employee per year. With 21 full-time employees, that comes to 3360 hours per year or 1.62 FTEs. The unit should either have a permanent staff of ~23 FTEs or should budget for temporary staff to cover vacations and prolonged illnesses.

Your patient is the 23-year-old female victim of an attempted abduction at knifepoint. The assailant was interrupted and drew his knife across the victim's throat as he fled the scene. A first responder on the scene immediately applied direct pressure to the neck wound. Your assessment reveals a very agitated young woman with ongoing blood loss despite direct pressure to the wound. There appears to be both bright-red bleeding and slower but steady dark-red bleeding. It appears that the trachea has not been penetrated but that there is a hematoma around the laceration. Which of the following is the most appropriate sequence of intervention for this patient?
A) Apply a pressure dressing to the wound, accompanied by pressure-point compression. B) Use rapid sequence intubation of the patient to protect the trachea from the expanding hematoma while maintaining direct pressure on the wound. C) Apply an occlusive dressing to the wound, and continue direct pressure. D) Apply direct pressure, and request an order from medical control to decompress the hematoma.
