Korotkoff sounds generate a clear, intense tapping in phase III. This is created as blood flows through the artery, but cuff pressure is still great enough to occlude flow during diastole. In phase I, the first audible sound is heard as the cuff pressure is released. It sounds like clear tapping and correlates to systolic pressure (the force needed to pump the blood out of the heart). The sound in phase II is like swishing or a murmur. It is created as the blood flows through blood vessels narrowed by the inflation of the blood pressure cuff. The sounds in phase IV are muffled and are heard when cuff pressure is low enough to allow some blood flow during diastole. The change from the tap of phase III to the muffled sound of phase IV is referred to as the first diastolic reading. In phase V, no sounds are heard. This occurs when cuff pressure is released enough to allow normal blood flow. This is referred to as the second diastolic reading.
