The presence of endogenous insulin is the major pathophysiologic distinction between type 1
and type 2 DM. In type 2 DM, the primary issue is insulin resistance in the tissues and the pancreas
usually continues to produce some insulin. However, the insulin that is produced is either insufficient
for the needs of the body and/or is poorly used by the tissues, which are insulin resistant. In type 1
DM, there is an absolute deficiency of endogenous insulin.
