A sentinel event is an unexpected occurrence involving death or serious physical or psychological injury. Example: amputating the wrong leg, significant medical errors, adverse drug reactions, confirmed transfusion reactions, surgery on the wrong patient or body part, infant abduction or discharge of infant to the wrong family.
