The liver is more refractory to hyperacute or acute rejection than other vascularized organs such as the kidney; low expression levels of HLA class I and the absence of HLA class II contribute to this refractory state. Donor and recipient still need to be matched for ABO, and transplant patients receive immunosuppressive therapy to control chronic rejection.
