Chart only in your area of responsibility.
Never use correction fluid on a chart or obscure any writing on a chart.
Chart in the proper order.
Use standard abbreviations and terms.
Do not write in the margins.
Never alter the information on a chart.
Never write information in a patient chart that you do not want the patient to see.
Chart anything the patient says with quotation marks.
Take the time to chart noncompliance especially well.
Be specific when charting.
If you see something on the chart that could be a potential problem, bring it to the attention of the dentist.
Make chart notations in ink.
Be sure that any change to patient's appointments is charted.
When charting appointments, include the reason for the appointment.
Make detailed notes of follow-up calls.
Update the patient's medical history at each visit.
