The nurse should include which of the following in the preoperative teaching plan for a patient with hyperparathyroidism who is scheduled to have a portion of his parathyroid gland removed?
a. Adhere strictly to a high-calcium diet.
b. Take over-the-counter supplements of vitamin D daily.
c. Maintain bed rest as much as possible.
d. Force fluids to at least 3,000 mL per day.
D
Hyperparathyroidism is caused by an overproduction of the parathyroid hormone, parathormone (PTH), from the parathyroid glands. PTH takes calcium from the bone and concentrates it in the blood. As a result, these patients are prone to recurrent nephrolithiasis or kidney stones. Forcing fluids dilutes the urine and prevents precipitation of stones. Remaining in bed contributes to stone formation. With an increased level of calcium in the blood, the patient should be on a low-calcium diet. Vitamin D would not be necessary to enhance the absorption of calcium.
