Particulate matter is any solid substance floating in an IV solution that should not be there. It can include rubber, glass, fibers, or undissolved drug. If the needle is not inserted correctly into the vial's stopper, rubber can break off and be transferred into the IV solution. Any time the rubber stopper breaks off into the vial, it is called coring. Many times when an ampule is opened, glass particles will fall to the bottom of the ampule and can be transferred into an IV solution if the preparer does not use a filter needle. Fibers can be transferred into an IV if the preparer opens a syringe, needle, or alcohol swab over an uncapped needle and syringe. The technician can also transfer undissolved drug into an IV if the additive is not allowed to properly dissolve before transfer. Such particulate matter injected into the vein can lodge and block blood flow or cause inflammation and possible infection (sepsis), as a foreign matter has invaded the body.
