In congenital adrenal hyperplasia (CAH), a genetic female develops ovaries normally as a fetus, but later in the course of prenatal development, the adrenal gland begins to function abnormally and produces an excess amount of androgens. Prenatal sexual differentiation then does not follow the normal female course. As a result, the external genitals are partly or completely male in appearance-the labia are partly or totally fused so there is no vaginal opening, and the clitoris is enlarged to the size of a small or full-sized penis.
In androgen-insensitivity syndrome (AIS), a genetic male produces normal levels of testosterone, but as a result of a genetic condition, the body tissues are insensitive to the testosterone and prenatal development is feminized. The individual is born with the external appearance of a female: a small vagina (but no uterus) and undescended testes.
