Answer should include: Information about the child's status, including physical health, cognitive development, communication development, social-emotional development, and adaptive development; If the family agrees to provide it, information about family needs, resources, and priorities related to the development of the child; Desired outcomes for the child and the family that should occur because of the early intervention program, the timelines for their occurrence, and criteria and procedures for determining their occurrence; The type of early intervention services that will be provided, the natural environments in which they will be provided, how often they will be provided, and who will provide them; Other services that will be provided, such as medical services; The dates services will be provided and the duration of services; The name of the service coordinator who will be responsible for implementing the IFSP and for coordinating all of the services to be provided; A transition plan to help the child and the family when it is time for the child to; move from the early interventions program into a preschool program.
