· The performing physician must collect and document the history of present illness because the performing physician may notice something in the patient's manner that may provide additional information or insight into the CC.
· The importance of this stems from the clinical knowledge, experience, training, and expertise of the performing physician in comparison with any other clinical staff member. And, since the performing physician will also be responsible for assessing and treating the patient, it is important that the performing physician understand the personal nature of this CC.
