• Miscommunication of drug orders. This type of error can occur with poor handwriting in the patient chart or on the prescription, confusion between drugs with similar names, misunderstanding of where zeroes and decimal points lie, confusion of metric with other dosing units, as well as or use of inappropriate abbreviations.
• Lack of appropriate labeling. This error happens most commonly when drugs are prepared or drawn up.
